（1）資格要求
Requirements for Qualification of Bidder
A．如果投標(biāo)人為境內(nèi)注冊公司，投標(biāo)人需提供合法有效的企業(yè)法人營業(yè)執(zhí)照、稅務(wù)登記證及組織機(jī)構(gòu)代碼證或證照合一的營業(yè)執(zhí)照，投標(biāo)時需提供原件掃描件（原件備查）；
如果投標(biāo)人為境外注冊公司，需提供有效的公司登記注冊證明；
A. The bidder within the customs territory shall provide valid independent legal person business license, tax registration certificate, organization code certificate, or combined certificate and business license. The scanned copy of the original shall be provided during bidding (the original copy for reference);
The bidder outside the customs territory shall provide valid certificate to prove valid business registration certificate.
B. 本次招標(biāo)不接受代理商投標(biāo)。
投標(biāo)人應(yīng)是投標(biāo)貨物的制造商（包括制造商所在集團(tuán)內(nèi)負(fù)責(zé)銷售的公司）。
如果投標(biāo)人是制造商所在集團(tuán)內(nèi)負(fù)責(zé)銷售的公司，應(yīng)提供聲明，說明投標(biāo)人與制造商隸屬同一集團(tuán)，負(fù)責(zé)投標(biāo)貨物的銷售工作。

B. Bid by Agent is not accepted. The bidder shall be the manufacturer of bid goods (including companies responsible for sales within the manufacturer's group). If the Bidder is a company in charge of sales within the manufacturer's group, a statement should be provided indicating that the Bidder and the manufacturer belong to the same group and are responsible for the sale of the bid Goods.

（2）資質(zhì)要求及證明文件 Requirements of Qualification of Bidder and Certificate
特種設(shè)備證書 ：投標(biāo)人須提供制造商的《中華人民共和國特種設(shè)備生產(chǎn)許可證》-壓力容器制造A2級及以上資質(zhì)，且證書在有效期內(nèi)。

Special Equipment License: Bidder shall provide manufacturer's “special equipment production license of the people's Republic of China” -Pressure Vessel Manufacturing Grade A2 and above Qualification Certificate, and the certificate shall be within the validity period.
（3）業(yè)績要求Requirement for Bidder's reference:
2009年1 月1 日至投標(biāo)截止日（以合同簽署時間為準(zhǔn)），投標(biāo)人所投產(chǎn)品的制造商應(yīng)具有至少1個合同的螺旋折流板換熱器的供貨業(yè)績，且須滿足設(shè)備直徑不小于2000mm、結(jié)構(gòu)形式為填料函式的技術(shù)要求。
（如果投標(biāo)人為制造商所在集團(tuán)內(nèi)負(fù)責(zé)銷售的公司，應(yīng)提供該制造商的業(yè)績）。

From January 1, 2009 to the deadline for submission of the bid (subject to the signing time of the contract), the manufacturer of the bidder’s bid goods shall have at least 1 contracts for supply performance of Helixchanger heat exchangers, and must meet the technical requirement that the diameter of the equipment is not less than 2000mm and the structure is of stuffing box type.（If the Bidder is a company in charge of sales within the manufacturer's group,?the?manufacturer's?performance?should?be?provided.
投標(biāo)人須按規(guī)定格式提交業(yè)績表，并提交相關(guān)業(yè)績證明文件。
業(yè)績證明文件包括但不限于：1）銷售合同復(fù)印件和2）到貨驗收材料。
投標(biāo)人所提交的業(yè)績證明文件必須至少體現(xiàn)以下內(nèi)容：合同簽署時間、合同簽字頁、制造商名稱、貨物名稱（螺旋折流板換熱器）、設(shè)備直徑、結(jié)構(gòu)形式（填料函式）及到貨驗收材料。

The Bidder shall furnish reference list in given form, and provide corresponding documentary evidence. The documentary evidence shall include but not limited to: 1) copies of sales contract and 2) acceptance evidence of goods delivery. The reference evidence documents furnished by Bidder must as least demonstrate the content as follows: date of signing contract, page?of?contract?signature,?name of manufacturer, name of Goods (Helixchanger heat exchanger), diameter of the equipment, type of structure (stuffing box) and acceptance evidence of goods delivery.
未提交業(yè)績證明文件，或所提供的業(yè)績證明文件無法體現(xiàn)合同簽署時間、合同簽字頁、制造商名稱、裝置名稱、貨物名稱、設(shè)計能力，均視為無效業(yè)績。
未提交業(yè)績證明文件，或所提供的業(yè)績證明文件無法體現(xiàn)合同簽署時間、制造商名稱、貨物名稱（螺旋折流板換熱器）、設(shè)備直徑、結(jié)構(gòu)形式（填料函式），均視為無效業(yè)績。

If no documentary evidence furnished, or documentary evidence furnished can not demonstrate date of signing contract, page?of?contract?signature,?name of manufacturer, name of Goods (Helixchanger heat exchanger), diameter of the equipment, type of structure (stuffing box), the relevant reference will be deemed invalid.
